11kV Ring Main Isolator (RMI) Endbox Failure

SP0548-1463 | On 4 May 2022, an 11kV Ring Main Isolator (RMI) Endbox fed out of Cronulla Zone Substation failed. There was no one in the vicinity at the time the failure occurred.

Uncontrolled Vehicle Movement

SP0548-1459 | Items placed on top of the handbrake pose a risk of releasing the handbrake allowing the vehicle to potentially move uncontrolled. This has the potential for a fatality, or to critically injure of a fellow worker.
